According to the CPSC notice, the identified hazard is: The recall involves the 200 and 300 Bar versions of the OMS Rotating Cross Bar Manifold. The manifold consists of two valves which have "O.M.S." stamped on the side of each valve and a crossbar that has three O-rings per side. Each part has a black plastic knob. Hazards: Undersized o-rings installed on the cross bar can cause a loss of breathing air from the SCUBA cylinder, presenting a risk of drowning.
