Reported risk: Issue:
On certain ambulances, the equipment support cushion installed at the end of the squad bench seat may not be properly attached. As a result, the cushion could detach and hit an occupant in a crash.
Safety Risk:
An equipment support cushion that hits an occupant could create a risk of injury.
Corrective Actions:
To reduce the safety risk, Demers advises that, when possible, you should avoid installing equipment on the cushion until the recall repairs have been completed. Demers will notify owners by mail and provide a repair kit and instructions for how to replace the equipment support cushion.
Verify each fact at the issuing agency before acting. Recall scope, affected lots, and remediation instructions may be updated by the agency after our last sync.
