Reported risk: Issue:
On certain aerial devices and digger derricks, the bolts for the pedestal and turntable may fail. If multiple bolts fail, the turntable and pedestal could separate from the vehicle.
Safety Risk:
If the pedestal and turntable separate from the vehicle, there may be a risk of injury.
Corrective Actions:
The company will notify owners by mail and advise them to perform a visual inspection of the rotation bearing bolts, looking for bolts marked with the number "5252". If any of the bolts are found broken or cracked, the unit should be taken out of service and the bolts should be replaced. Bolts marked with the number 5252 that are not broken or cracked must be replaced at the next annual service.
