Reported risk: The Series 503 Flushmate® III system can burst at or near the vessel weld seam releasing stored pressure. This pressure can lift the tank lid and shatter the tank, posing impact and laceration hazards to consumers and property damage. Flushmate has received two reports of the units bursting, resulting in property damage, including one report of a minor injury in the United States. Flushmate has received six reports of the units bursting in Canada, all related to the original June 2012 recall. No injuries were reported. Neither Flushmate nor Health Canada has received any reports of incidents or injuries to Canadians related to the use of the products included in this recall notice.
